There will come a time when you’ll need to make an insurance claim. So how do you go about it to ensure you get your money hassle free? Here are some tips.
Medical Claims
If you are making a hospitalisation and surgery (H&S) claim, the documents that are generally required are:-
- claim form, signed by the claimant
- medical report by the doctor(s) who attended to you
- any related lab report, blood test, x-rays that were done
- itemised bills and receipts
For Critical Illness (CI) claim, it depends on the type of claim. For example, the documents required for a cancer claim is:-
- claim form, signed by the claimant
- medical report by the doctor(s) who attended to you
- any related lab report, blood test, x-rays that were done
- usually a biopsy result
Accident Claim
Accidental claim usually means a claim involving accidental injuries, temporary disability, lost of limbs or lost of use of limbs. The document required are similar to a H&S claim above with the added police report if there is one like in the case of a car accident.
Death Claim
Required documents for a death claim include
- claim form, signed by the claimant
- death certificate
- deceased proof of age i.e NRIC or birth cert
- the original insurance policy
- proof of relationship i.e. birth cert or marriage cert
- autopsy report if death is accidental in nature
